Varicoceles: The Diagnostic Dilemma

The treatment of clinical and subclinical varicoceles in the male subfertile population has been much debated over the last several decades. Unfortunately, regardless of whether you believe that there is or isn't a benefit, there are currently no strict diagnostic criteria used to evaluate varicoceles. Whether by physical examination alone, or in conjunction with imaging modalities such as Colour-Doppler ultrasound, diagnosing a varicocele has been shown to have significant inter- and intra-observer variability. If we are to decipher whether repairing varicoceles is truly beneficial, we must first be able to consistently and accurately diagnose, compare, and categorize varicoceles. Without standardized, valid and reliable measurement tools, the role of varicocelectomy in the infertile male will remain uncertain.
